How do you know which diseases are actually demons?

I would say by the Spirit of God. Clearly, some sicknesses are according to the Scriptures. I am not sure that there is a pattern laid out. Is that what you mean?

I guess I am asking is there a pattern. Are some illnesses always demonic? Is there such a thing as a lifestyle diseases (diabetes, cancer, etc.)?
Should we always approach illnesses and demonic attacks?

Well, some ministers (including ones I respect) have taught that there is divine healing in Jesus for all illnesses. I believe that such is possible, but am not sure that I would agree about divine healing being a right for all Christians.
As you know, I was diagnosed with cancer about 2 years ago. Initially, I struggled with these types of questions. "God", I would ask, "I thought you said that you wouldn't put any of these curses on us if we obeyed you?"
But, you want to know what I came to find out? Nothing. :-) I really didn't get an answer to that. I eventually found peace simply trusting God to be God and knowing that He will always work for my good.
I definitely don't think that illness is part of God's best for us, but I believe He can use it to further perfect us according to His purposes, while we are in these temporal bodies.
I would say that we should seek deliverance from all that ails us, whether physical or spiritual. If God does not remove it, He will give us strength to endure. :-)
